Roles & Responsibilities

• Conduct comprehensive OASIS assessments and complete accurate home health documentation
• Manage and coordinate patient care plans as the Home Health RN Case Manager
• Provide skilled nursing visits, clinical assessments, and patient education in the home
• Collaborate with interdisciplinary team members and community resources to ensure continuity of care
• Communicate clearly with patients and families to support goals and safety in the home environment
• Maintain compliance with home health regulations and agency policies while promoting quality outcomes

Qualifications

• Registered Nurse (RN) with active licensure (ability to practice in Vermont preferred)
• OASIS experience required
• Home health and case management experience required
• Strong documentation and clinical assessment skills
• Prefer an experienced travel nurse
• Availability for the full contract period with no requested time off

Education

• Graduate of an accredited nursing program (ADN or BSN) and current RN licensure

Knowledge & Skills

• Proficiency with OASIS assessments and home health documentation
• Case management and care coordination skills
• Skilled nursing assessment and clinical decision-making
• Patient and family education in the home setting
• Comfortable using electronic medical records and clear clinical communication
• Time management and ability to manage a caseload independently
