Roles & Responsibilities

Evaluate and develop personalized treatment plans for residents in a skilled nursing setting
Help patients regain mobility, strength, and independence through hands-on therapy
Collaborate with an interdisciplinary team to deliver the best possible care
Document progress and adjust interventions to meet each patient's goals
Provide compassionate support and encouragement every step of the way

Qualifications

Current PT or DPT license in Minnesota
New graduates are welcome to apply
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced SNF environment

Knowledge & Skills

Skilled in physical therapy assessment and treatment techniques
Comfortable working with geriatric populations
Able to adapt quickly to changing patient needs
Team-oriented with excellent documentation abilities
