Physical Therapist - Home Health
Location: Dickinson, North Dakota
Start Date: 06-08-2026

Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes
- Conduct start of care assessments and develop personalized treatment plans
- Perform recertifications and resumption of care visits as needed
- Complete 6.5 points per day including regular visits and admissions
- Collaborate with clinical coordinators and sister agency therapists for support
- Document patient progress using OASIS standards
- Ensure safe transitions of care and patient education
Qualifications
- Current ND Physical Therapy License required
- BLS certification required
- Valid Driver's License and Auto Insurance required
- Home Health experience preferred
- OASIS training preferred
- First time travelers welcome
Knowledge & Skills
- Physical Therapy expertise in home health settings
- OASIS documentation proficiency
- Strong communication and patient education skills
- Ability to work independently in the field
- Time management for 5-6 visits per day
