Physical Therapist - Home Health
Location: Red Bluff, California

Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Deliver personalized physical therapy care to patients in the comfort of their homes
- Perform Start of Care visits especially with orthopedic patients when nursing is not ordered
- Help patients recover mobility and independence after joint replacements and other orthopedic procedures
- Support geriatric patients managing multiple chronic conditions like CHF and COPD
- Document visits accurately using the HomeCare HomeBase EMR system on your provided tablet
- Collaborate with the intake RN and manager to coordinate seamless patient care
Qualifications
- Active Physical Therapist license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years if they have Home Health background
- Comfortable working independently in home settings
- Valid driver's license and reliable transportation
Knowledge & Skills
- Strong assessment and clinical reasoning skills for diverse patient populations
- Excellent communication and patient education abilities
- Proficiency with EMR documentation, especially HomeCare HomeBase
- Ability to manage a caseload of approximately 5 visits per day
- Comfort navigating rural service areas with an average of 40 miles daily
