Roles & Responsibilities
- Deliver personalized physical therapy in patients' homes to help them regain strength and independence
- Create customized treatment plans that fit each patient's unique goals and living environment
- Collaborate with physicians, nurses, and family members to ensure seamless care coordination
- Document progress and adjust therapy techniques to maximize patient outcomes
- Educate patients and caregivers on safe exercises and mobility techniques
Qualifications
- Current California Physical Therapist license required
- BLS certification required
- Home health experience is a plus but not mandatory
- Strong communication skills and a passion for patient-centered care
Knowledge & Skills
- Expertise in therapeutic exercise and functional mobility training
- Ability to assess and treat a variety of orthopedic and neurological conditions
- Comfortable working independently in home settings
- Solid documentation and time-management abilities
