Roles & Responsibilities

Provide compassionate physical therapy services across four rural counties
Conduct comprehensive Oasis Assessments and Start of Care evaluations
Help elderly patients recover from falls and fractures with personalized treatment plans
Manage a manageable caseload of 4-6 patient visits daily using our point system
Document visits and track time efficiently with provided tablet technology
Build meaningful relationships with patients in their home environments

Qualifications

Current Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care documentation
Comfortable working independently in rural settings
Strong assessment and clinical reasoning skills
Reliable transportation for travel between counties

Knowledge & Skills

Physical Therapy clinical expertise
Oasis Assessment proficiency
Start of Care evaluation experience
Rural healthcare delivery understanding
