Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Deliver compassionate physical therapy care to patients in their homes across greater Sacramento county
- Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
- Help patients regain mobility and independence with personalized treatment plans
- Collaborate with interdisciplinary teams to ensure the best outcomes for every patient
- Document visits accurately and maintain productivity goals of five to six patients per day
Qualifications
- Active Physical Therapy Assistant license in California
- Prior home health experience preferred
- New grads considered only in exceptional circumstances
- Strong communication and time-management skills
- Ability to work independently in a home setting

- Home health care protocols
- Patient assessment and treatment planning
- Cardio, pulmonary, orthopedic, neurological, and geriatric care
- Accurate documentation and mileage tracking
- Building rapport with patients and families
