Roles & Responsibilities

• Evaluate residents and perform skilled physical therapy assessments to identify functional needs
• Develop and implement individualized treatment plans that improve mobility and independence
• Deliver evidence-based therapeutic interventions such as gait training, balance work and therapeutic exercise
• Document clinical progress clearly and timely to support quality care and reimbursement
• Collaborate with nursing and interdisciplinary team members to coordinate resident-centered care
• Educate residents and families on safe movement strategies and home programs to sustain gains

Qualifications

• PT or DPT credential (Primary skills listed: PT, DPT)
• Any experience level considered, including new graduates — bring your drive and we’ll support your start
• Comfort working in a skilled nursing facility setting (SNF)
• Strong communication and documentation skills to work effectively with residents and the care team
• Current CPR/BLS certification as required by facility policies

Education

• DPT preferred; PT credentials acceptable
• New graduates welcome

Knowledge & Skills

• Skilled in resident assessment, mobility and functional restoration
• Experience with therapeutic exercise, balance and gait training techniques
• Good understanding of SNF workflows and documentation expectations
• Ability to collaborate with nursing, social work and rehab team members
• Strong patient education and family coaching skills
