Roles & Responsibilities

• Perform thorough evaluations to assess patients' physical abilities and set meaningful goals
• Design and implement individualized treatment plans to improve mobility, strength, and independence
• Deliver hands-on therapy and therapeutic modalities tailored to each resident's needs
• Educate patients and families on exercises, safe movement techniques, and home modifications
• Collaborate with occupational therapists, nurses, and physicians to coordinate comprehensive care
• Monitor progress and adjust treatment plans to ensure optimal outcomes
• Maintain accurate, timely clinical documentation in accordance with facility policies and regulations

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in California (state license required)
• Experience in a skilled nursing facility (SNF) or similar setting preferred
• Strong knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work independently and as part of an interdisciplinary team
• Compassionate, patient-focused approach to care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy (from an accredited program)

• Rehabilitation and mobility improvement strategies
