Contract Type: Travel Contract

Shift Type: Day shifts, Monday-Friday, no weekends or holidays

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct initial start-of-care visits and comprehensive assessments for orthopedic and geriatric patients
Develop and implement individualized physical therapy treatment plans in a home health setting
Collaborate with an interdisciplinary team including nurses and therapists to ensure coordinated care
Document patient progress using HCHB (HomeCare HomeBase) EMR system
Educate patients and families on therapeutic exercises and safety strategies

Qualifications

Active Physical Therapy license in California
Current BLS certification
Minimum 2 years of experience (will consider less with strong home health background)
Home Health experience required
Valid driver's license and reliable transportation

Knowledge & Skills

Proficient in HCHB or similar home health EMR
Strong clinical skills in orthopedics and geriatrics
Excellent communication and patient education abilities
Ability to manage a daily caseload of approximately 5 visits
Self-motivated and comfortable working independently in the field
