Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care, recertifications, and resumption of care visits for home health patients
Perform 6-7 point-of-care visits per day, including evaluations and follow-ups
Collaborate with clinical coordinators and sister agency therapists for support
Complete accurate OASIS documentation and maintain patient records
Provide patient and family education on treatment plans and safety
May occasionally assist with hospice safety evaluations or teaching visits

Qualifications

Current and active North Dakota Physical Therapy license
BLS certification required
Minimum 1 year of home health experience preferred
OASIS training and experience preferred
Valid driver's license and auto insurance required
Ability to travel within a 50-mile radius

Knowledge & Skills

Proficient in home health and hospice care principles
Skilled in OASIS documentation and point-of-care systems
Strong clinical assessment and critical thinking abilities
Excellent communication and patient education skills
Ability to work independently and manage a daily caseload
