Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Perform comprehensive Oasis Assessments and Start of Care visits
Develop and implement patient-centered physical therapy plans
Manage a caseload of 4-6 patient visits daily using a point system
Document visits and track time using a tablet
Provide care for elderly patients recovering from falls and fractures
Collaborate with a multidisciplinary team across four counties

Qualifications

Active Physical Therapist license in Indiana
BLS/CPR certification
Home health experience preferred
Comfortable with Oasis Assessments and Start of Care documentation
Valid driver's license and reliable transportation

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Excellent communication with patients and families
Time management and ability to work independently
Proficiency with tablet-based EMR systems
Knowledge of home health regulatory requirements
