Licensed Practical Nurse (LPN) - Long Term Care

Location: Breckenridge, MN

Start Date: 07-06-2026

End Date: 10-05-2026

Contract Type: Travel Contract

Shift Type: Cycle scheduling with rotating shifts: days (6:00 AM - 2:30 PM), evenings (2:00 PM - 10:30 PM), and nights (10:00 PM - 6:30 AM); every other weekend required

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide compassionate care to residents in a long-term care setting
Administer medications and treatments as prescribed
Monitor patient vitals and document changes in PointClickCare EMR
Collaborate with RNs and CNAs to ensure quality care
Assist with daily living activities and promote resident independence
Maintain a safe and clean environment for residents

Qualifications

Current LPN/LVN license in MN or compact state
BLS certification (required)
Experience with PointClickCare EMR preferred
Strong teamwork and communication skills
Willingness to work every other weekend and holiday

Knowledge & Skills

Proficient in PointClickCare electronic medical records
Knowledge of CHF, COPD, Alzheimer's, Parkinson's, and diabetes care
Ability to work in a fast-paced, team-oriented environment
Strong work ethic and dedication to patient well-being
