Roles & Responsibilities

Evaluate patients in the home setting and develop individualized, evidence-based treatment plans
Provide skilled physical therapy services to improve mobility, strength, balance, and functional independence
Educate patients and caregivers on exercises, safety strategies, and injury prevention in a clear, compassionate way
Document evaluations, progress notes, and outcomes accurately and in a timely manner
Collaborate with physicians, nurses, and interdisciplinary care teams to coordinate high-quality care

Qualifications

Active Physical Therapist (PT) license in the state of Iowa
Graduate of an accredited Physical Therapy program
Home Health experience preferred but not required — we welcome motivated clinicians ready to learn
Strong clinical judgment, time management, and communication skills
Ability to work independently while delivering compassionate, patient-centered care

Education

Graduate of an accredited Physical Therapy program
