Roles & Responsibilities

• Assess patients’ physical function and develop individualized treatment plans that make a real difference
• Implement therapeutic interventions and evidence-based modalities to improve mobility, strength, and independence
• Document patient progress clearly and timely in the patient record
• Collaborate with nurses, physicians, and interdisciplinary team members to coordinate excellent care
• Provide patient and family education to support recovery and safe discharge
• Monitor outcomes and adjust treatment plans to maximize patient progress
• Ensure compliance with licensing and facility policies, including infection control and safety procedures

Qualifications

• Active Physical Therapist License (state license verification required)
• BLS (Basic Life Support) certification
• Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist as required
• Successful completion of license/certificate verification and required health screenings
• Willingness to complete Trinity 10-panel drug screen, Fit Test, and required immunizations/documentation

Knowledge & Skills

• Strong knowledge of therapeutic interventions, mobility training, and rehabilitation techniques
• Excellent clinical assessment and documentation skills
• Clear, compassionate communication with patients, families, and care teams
• Ability to prioritize care and manage caseload efficiently
• Familiarity with facility policies for safety, infection control, and patient handling
