Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $57 per hour

Roles & Responsibilities

Deliver personalized occupational therapy services in patients' homes across assigned geographic regions
Develop and implement individualized treatment plans for patients recovering from strokes, orthopedic injuries, joint replacements, and spinal conditions
Collaborate with PT and Speech colleagues to provide comprehensive home-based rehabilitation
Complete OASIS documentation accurately to meet government-mandated requirements
Support patients and families with education and resources to promote independence and safety at home

Qualifications

Active Occupational Therapist license in California
BLS certification required
Minimum one year of experience working as an OT
Experience with OASIS documentation required
Home health setting experience preferred but not required

Knowledge & Skills

Strong understanding of OASIS documentation requirements
Ability to work independently in home-based care settings
Excellent communication and patient education skills
Comfortable with Cerner EMR system
Flexible and adaptable approach to scheduling and patient needs
