Roles & Responsibilities

• Provide compassionate, hands-on nursing care as an LPN to residents and patients
• Administer medications and treatments safely and accurately following facility protocols
• Monitor patient condition and communicate changes promptly to RN and care team
• Document assessments, interventions, and patient responses clearly and promptly
• Collaborate with interdisciplinary team members to create and follow individualized care plans
• Assist with activities of daily living and support patient comfort and dignity
• Participate in mandatory trainings and orientation to maintain high standards of care

Qualifications

• Current, valid LPN license (required)
• Active or eligible to obtain Pennsylvania nursing license
• Basic Life Support (BLS/CPR) certification preferred or required per facility policy
• Excellent communication skills and the ability to work within a care team
• Comfortable with electronic charting and accurate documentation
• Dependable, professional, and patient-centered attitude

Education

• Completion of an accredited LPN/LVN program or vocational nursing program

Knowledge & Skills

• Fundamental nursing assessment and clinical observation skills
• Medication administration and basic IV/site care knowledge (as applicable)
• Familiarity with infection control and patient safety practices
• Ability to prioritize tasks and manage time during busy shifts
• Strong interpersonal skills for working with patients, families, and staff
• Basic computer skills for electronic health records and documentation
