Roles & Responsibilities
- Provide compassionate, resident-centered nursing care and support daily living activities
- Administer medications and treatments safely and according to policy
- Accurately document assessments, interventions, and patient responses
- Monitor residents for changes in condition and report promptly to the care team
- Collaborate with nurses, therapists, and support staff to create a seamless care experience
- Assist with admissions, transfers, and discharges to ensure continuity of care
- Maintain a safe, clean environment and follow infection control practices
Qualifications
- Current LPN license in Pennsylvania (active and in good standing)
- CPR/BLS certification (or willingness to obtain upon hire)
- Proof of required immunizations and TB screening as per facility policy
- Ability to pass background check and drug screen
- Previous experience in long-term care or skilled nursing preferred
Education
- Vocational nursing diploma or LPN certificate (or equivalent)
Knowledge & Skills
- Solid clinical assessment and observation skills
- Medication administration and basic wound care knowledge
- Comfortable with electronic health record documentation
- Strong communication and teamwork abilities
- Good time management and prioritization in a busy care setting
- Compassionate patient/resident interaction and dignity-focused care
