Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care and support daily living activities
Administer medications and treatments accurately and on schedule
Perform patient assessments and report changes promptly to the care team
Document patient care and observations clearly in the medical record
Collaborate with nurses, CNAs, and interdisciplinary staff to deliver coordinated care
Assist with admissions, discharges, and transfers to ensure smooth transitions
Maintain a safe, clean environment and follow infection control practices

Qualifications

Current, active LPN license (able to practice in Pennsylvania preferred)
BLS/CPR certification or willingness to obtain upon hire
Completion of an accredited LPN program or vocational nursing certificate
Comfortable administering medications, performing assessments, and documenting care
Strong interpersonal and communication skills; patient-focused attitude
Previous experience in long-term care or skilled nursing is a plus

Education

LPN diploma or vocational nursing certificate
High school diploma or GED

Knowledge & Skills

Medication administration and basic clinical assessments
Charting and electronic health record familiarity
Infection control and safe patient handling practices
Time management and prioritization in a busy care setting
Teamwork and clear communication with patients and staff
Ability to follow protocols and escalate concerns appropriately
