Roles & Responsibilities

Provide compassionate, high-quality nursing care to assigned residents
Administer medications and treatments safely and accurately
Assess, monitor and document residents’ clinical status and vital signs
Collaborate with the interdisciplinary team to develop and follow care plans
Communicate clearly with residents and families to support positive outcomes
Maintain accurate, timely documentation in the electronic medical record

Qualifications

Valid LPN license in Pennsylvania (required)
Completion of an accredited LPN/LVN program
Current BLS (Basic Life Support) certification preferred or willing to obtain
Ability to perform typical LPN duties including medication administration and documentation
Strong communication skills and a team-oriented mindset
Previous long-term care or clinical nursing experience is a plus

Education

Graduate of an accredited LPN/LVN program
High school diploma or GED

Knowledge & Skills

Skilled in medication administration and basic nursing procedures
Knowledge of infection control best practices
Comfortable with electronic medical records and documentation
Strong observational skills for monitoring resident condition changes
Able to prioritize tasks in a fast-paced clinical environment
