Roles & Responsibilities
- Provide safe, compassionate direct nursing care to residents and patients
- Administer medications and treatments accurately and document thoroughly
- Monitor and record vital signs, reporting changes to the care team promptly
- Collaborate with RNs, CNAs, and interdisciplinary staff to support individualized care plans
- Maintain clear, timely charting and documentation to ensure continuity of care
- Support residents' activities of daily living and promote dignity and independence
- Participate in infection control practices and maintain a safe care environment
Qualifications
- Active Connecticut LPN license required
- Current BLS/CPR certification required
- Comfortable with medication administration, wound care basics, and routine nursing tasks
- Strong communication and teamwork skills; compassionate bedside manner
- Experience in long-term care or skilled nursing preferred but not required
Education
- Vocational nursing diploma or equivalent (LPN program)
- High school diploma or GED
Knowledge & Skills
- Clinical assessment and basic nursing interventions
- Medication administration and documentation best practices
- Resident-centered care and safety-focused procedures
- Effective communication with residents, families, and clinical teams
- Charting in electronic health records and accurate documentation
