Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents according to plan of care
• Administer medications and treatments accurately and document care in the medical record
• Assess resident condition regularly and report changes to the care team
• Collaborate with nurses, therapists, and care staff to deliver coordinated care
• Assist with activities of daily living while promoting resident dignity and independence
• Participate in care planning, rounding, and family communication as needed
• Follow infection control, safety policies, and facility protocols consistently

Qualifications

• Current LPN license (practical/vocational nursing) — licensure appropriate for Connecticut preferred
• Completion of an accredited practical nursing program
• Basic Life Support (BLS/CPR) certification or willingness to obtain
• Comfortable working in a long-term care/skilled nursing environment; prior LTC experience a plus
• Strong communication skills and a team-first attitude
• Ability to pass required background checks and health screenings

Education

• Graduation from an accredited practical nursing (LPN/LVN) program

Knowledge & Skills

• Medication administration and documentation best practices
• Wound care, catheter care, and basic clinical assessment skills
• Knowledge of infection prevention and safety protocols
• Familiarity with electronic medical records or willingness to learn
• Time management and the ability to prioritize resident needs
• Compassionate bedside manner and resident-centered care approach
