Roles & Responsibilities

• Provide compassionate, hands-on patient care and support activities of daily living
• Administer medications and treatments accurately and according to facility policy
• Perform timely patient assessments and document findings in the medical record
• Collaborate with RNs and the interdisciplinary team to coordinate excellent care
• Communicate clearly with patients and families about care needs and changes
• Monitor patient conditions and escalate concerns promptly to the appropriate clinician
• Participate in unit safety and quality improvement activities to enhance patient outcomes

Qualifications

• Current LPN license (Georgia license preferred or eligibility to obtain one)
• Completion of an accredited LPN/LVN program
• Basic Life Support (BLS) certification required
• Prior hands-on nursing experience preferred; experience in long-term care or acute care is a plus
• Ability to pass background check and meet facility immunization requirements

Education

• Vocational Nursing diploma or equivalent
• Graduate of an accredited LPN/LVN program

Knowledge & Skills

• Patient assessment and vital signs monitoring
• Medication administration and accurate documentation
• Wound care basics and infection control practices
• Charting and electronic health record familiarity
• Strong communication, teamwork, and time management skills
• Ability to prioritize tasks in a busy clinical environment
