Roles & Responsibilities
- Provide compassionate, patient-centered nursing care within scope of practice
- Administer medications and treatments accurately and safely
- Monitor and document patient condition, reporting changes to the care team
- Collaborate with nurses, aides, and interdisciplinary team members to deliver quality care
- Assist with ADLs and support patients' comfort, dignity, and mobility
- Participate in care planning and ensure timely, thorough charting
- Support infection control and safety protocols to protect patients and staff
Qualifications
- Active LPN license in Pennsylvania (required)
- Current BLS/CPR certification (recommended or required by facility)
- Completion of an accredited LPN/LVN program (diploma or certificate)
- Comfort with medication administration, wound care basics, and patient assessments
- Positive communication skills and ability to work well on a collaborative care team
- Preferred: prior experience in long-term care, skilled nursing, or similar setting
Education
- Graduate of an accredited practical nursing program (LPN/LVN diploma or certificate)
Knowledge & Skills
- Strong medication administration and documentation skills
- Familiarity with electronic health records and accurate charting
- Basic wound care, vital signs monitoring, and patient assessment
- Infection control best practices and safety awareness
- Good time management and ability to prioritize patient needs
- Empathetic bedside manner and effective communication with patients and families
