Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents, prioritizing safety and dignity
- Administer medications and treatments accurately and document thoroughly
- Monitor patient conditions and communicate changes promptly to the care team
- Collaborate with RNs, CNAs, and interdisciplinary teams to create and follow individualized care plans
- Support resident activities of daily living and encourage independence when possible
- Maintain accurate electronic and paper records in line with facility policy
- Participate in shift handovers and contribute to a positive, team-first culture
Qualifications
- Current LPN license in Pennsylvania (or compact/state equivalent) — required
- BLS/CPR certification — required or willing to obtain
- Prior experience in long-term care, assisted living, or similar settings preferred
- Strong verbal and written communication skills and a patient-focused approach
- Ability to pass required background checks and health screenings
Education
- Graduate of an accredited LPN/LVN program (diploma or certificate)
- High school diploma or GED
Knowledge & Skills
- Solid knowledge of medication administration and basic nursing procedures
- Comfortable with electronic health record documentation
- Good clinical assessment skills and ability to escalate concerns appropriately
- Excellent time management and prioritization in a busy clinical environment
- Team-oriented mindset with strong interpersonal skills
