Roles & Responsibilities

• Provide skilled, compassionate nursing care to assigned patients as an LPN
• Administer medications and treatments according to physician orders and facility policy
• Monitor and document patient status, reporting changes promptly to the care team
• Assist with daily living activities and implement individualized care plans
• Collaborate with RNs, providers, and multidisciplinary staff to ensure continuity of care
• Maintain accurate, timely documentation in the patient record system
• Adhere to infection control, safety protocols, and facility policies at all times

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• Current Basic Life Support (BLS) or CPR certification (or ability to obtain quickly)
• Clinical experience in a relevant care setting preferred (long-term care, med-surg, or rehab)
• Ability to pass required background checks and meet facility employment requirements
• Reliable transportation and flexibility to work per-diem shifts at the Wilkes Barre facility

Education

• Graduate of an accredited Practical Nursing program (diploma/certificate)

Knowledge & Skills

• Strong medication administration and documentation skills
• Comfortable with patient assessments and reporting clinical changes
• Good communication skills with patients, families, and the care team
• Ability to prioritize tasks in a fast-paced clinical environment
• Basic wound care, catheter care, and ADL assistance skills
• Familiarity with electronic health record systems is a plus
• Dependable, detail-oriented, and patient-focused approach to care
