Roles & Responsibilities
- Provide compassionate bedside care and daily support to patients and residents
- Administer medications and perform treatments according to care plans and orders
- Perform thorough patient assessments and promptly report any changes to the team
- Document patient care accurately and maintain clear records in the charting system
- Collaborate with RNs, physicians, and interdisciplinary staff to deliver coordinated care
- Assist with activities of daily living and support patient mobility and comfort
- Contribute to quality improvement and infection control initiatives on the unit
Qualifications
- Current LPN license in Pennsylvania (required)
- Basic Life Support (BLS/CPR) certification preferred or required per facility
- Completion of an accredited Practical Nursing program
- Comfortable with electronic charting and basic clinical documentation
- Strong communication skills and a team-first attitude
- Current immunizations and facility-required health screenings
Education
- Graduate of an accredited Practical Nursing program
- Current LPN licensure in Pennsylvania
Knowledge & Skills
- Medication administration and safe documentation practices
- Patient assessment and basic clinical judgment
- Infection prevention and safety procedures
- Time management and ability to prioritize patient needs
- Clear, compassionate communication with patients, families, and team members
- Familiarity with electronic health record systems
