Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and monitoring
• Perform timely and accurate nursing assessments and document findings in the chart
• Collaborate with RNs, CNAs, therapists, and the interdisciplinary team to deliver coordinated care
• Implement care plans and report changes in patient condition promptly
• Assist with ADLs and ensure patient comfort, safety, and dignity
• Educate patients and families on care plans, medications, and follow-up needs
• Support quality improvement efforts and maintain accurate, compliant documentation

Qualifications

• Current LPN license (required) — ability to practice in Pennsylvania preferred
• Recent experience in skilled nursing or long-term care settings is highly desirable
• Basic Life Support (BLS) certification or willingness to obtain
• Strong clinical judgment, solid medication administration skills, and attention to detail
• Excellent communication skills and a team-first attitude
• Ability to stand, walk, and assist patients as needed for typical shift duties

Education

• Practical Nursing diploma or equivalent; LPN program graduate required

Knowledge & Skills

• Knowledge of basic nursing principles, infection control, and patient safety practices
• Familiarity with electronic health records and accurate charting
• Skill in medication administration and patient monitoring
• Ability to prioritize tasks in a fast-paced care setting
• Strong interpersonal skills for working with residents, families, and staff
