Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
• Administer medications and treatments safely and accurately according to facility protocols
• Monitor and document vital signs, observations and changes in condition in the medical record
• Assist residents with activities of daily living (ADLs) while promoting dignity and independence
• Collaborate with RNs, therapists and the interdisciplinary team to implement individualized care plans
• Communicate clearly and kindly with families and caregivers about resident status and needs
• Maintain infection control standards and facility policies to keep residents and staff safe

Qualifications

• Active LPN license in Pennsylvania (or compact/reciprocal where applicable)
• Graduation from an accredited practical nursing program
• Current BLS/CPR certification (or willingness to obtain promptly)
• Experience in long-term care or skilled nursing preferred but new grads with clinical experience are welcome
• Ability to pass background checks, drug screening and meet facility immunization requirements
• Reliable, compassionate, and able to work collaboratively in a busy care environment

Education

• High school diploma or GED
• Completion of an accredited LPN/Vocational Nursing program
• Active LPN license required

Knowledge & Skills

• Strong medication administration and documentation skills
• Basic clinical assessment and resident observation abilities
• Familiarity with electronic health records (EHR) or readiness to learn
• Infection control best practices and safe patient handling
• Excellent communication, time management and teamwork skills
• Comfort assisting with wound care, ADLs and routine nursing procedures
