Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a respectful manner
• Administer medications and treatments safely and document accurately
• Monitor patient conditions and report changes promptly to the care team
• Follow individualized care plans and assist with activities of daily living
• Collaborate with nurses, therapists, and other staff to deliver coordinated care
• Keep accurate clinical documentation and support continuity of care
• Practice infection control and safety protocols to protect residents and staff
• Participate in shift handovers and help maintain a positive unit culture

Qualifications

• Active LPN license valid in Pennsylvania
• Graduation from an accredited practical nursing program
• Current BLS/CPR certification or willingness to obtain upon hire
• Strong communication skills and a patient-centered mindset
• Comfortable with medication administration, charting, and basic wound care
• Ability to pass background check and required health screenings
• Previous long-term care or skilled nursing experience is a plus

Education

• Completion of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic nursing assessments: vitals, skin integrity, pain checks
• Familiarity with electronic health records and accurate charting
• Infection control standards and safe patient handling
• Teamwork, time management, and prioritization under busy conditions
• Clear, compassionate communication with residents and families
