Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents in accordance with state regulations and facility policy
• Administer medications and treatments accurately while documenting in the chart
• Assess patient condition, monitor vital signs, and report changes promptly to the care team
• Collaborate with RNs, CNAs, therapists, and families to create and follow individualized care plans
• Maintain a safe, clean, and respectful environment for residents
• Support admission, discharge, and transfer processes with clear communication and thorough documentation

Qualifications

• Valid LPN license in Pennsylvania (or compact license where applicable)
• Current BLS/CPR certification (or ability to obtain upon hire)
• Prior experience in long-term care, skilled nursing, or similar setting preferred
• Strong communication skills and the ability to work well within a multidisciplinary team
• Ability to safely lift/move patients per facility guidelines and complete required documentation

Education

• Practical nursing diploma or equivalent from an accredited program

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and restorative nursing principles
• Familiarity with electronic medical records and charting
• Clinical assessment skills including vital signs and recognizing changes in status
• Time management and prioritization in a fast-paced care environment
• Customer-service oriented approach when interacting with residents and families
