Roles & Responsibilities

• Provide compassionate direct patient care that keeps residents comfortable and safe
• Administer medications and treatments accurately and according to facility policy
• Monitor and document vital signs, changes in condition, and clinical observations
• Collaborate with nurses, CNAs, and interdisciplinary team members to deliver coordinated care
• Assist with admissions, discharges, and transfers with attention to detail and dignity
• Maintain accurate, timely patient records and complete required documentation
• Follow infection control protocols and promote a clean, therapeutic environment

Qualifications

• Active LPN license in Pennsylvania (required)
• Completion of an accredited LPN/Vocational Nursing program
• Current BLS (Basic Life Support) certification or willingness to obtain
• Previous hands-on clinical experience as an LPN is preferred
• Strong communication skills and a collaborative, team-first attitude
• Ability to pass any required facility screenings and background checks

Education

• Graduation from an accredited LPN/Vocational Nursing program
• High school diploma or GED

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic nursing assessments and patient monitoring
• Familiarity with long-term care routines and resident-centered care
• Electronic medical record (EMR) documentation comfort
• Infection control and safe patient handling principles
• Time management and prioritization in a busy clinical setting
