Roles & Responsibilities

• Provide compassionate, resident-centered nursing care consistent with LPN scope of practice
• Administer medications and treatments accurately and document per facility policy
• Monitor and record vital signs, observe changes in condition, and report findings promptly
• Support activities of daily living and promote patient comfort and dignity
• Collaborate with RNs, CNAs, and interdisciplinary team members to coordinate care
• Maintain accurate and timely electronic health record documentation
• Adhere to infection control standards and facility policies to ensure a safe environment
• Participate in quality improvement initiatives and resident care planning

Qualifications

• Active LPN license in Pennsylvania
• Current BLS/CPR certification (or ability to obtain prior to start)
• Completion of an accredited practical nursing program
• Ability to pass background check and drug screen per facility requirements
• Comfortable working in a long-term care/skilled nursing environment; prior experience preferred but not required
• Strong communication skills and a team-first attitude

Education

• Graduation from an accredited practical nursing (LPN) program
• High school diploma or GED

Knowledge & Skills

• Medication administration and basic nursing assessments
• Familiarity with electronic health records and clinical documentation
• Basic wound care and infection prevention practices
• Time management and prioritization in a fast-paced clinical setting
• Effective communication with residents, families, and interdisciplinary teams
• Ability to follow care plans and escalate changes in condition appropriately
