Roles & Responsibilities

• Provide high-quality, compassionate nursing care to residents based on individualized care plans
• Administer medications and treatments accurately and document thoroughly
• Monitor resident condition, report changes to interdisciplinary team, and escalate when needed
• Perform assessments, support ADLs, and promote resident dignity and independence
• Collaborate with nurses, CNAs, therapists, and leadership to ensure safe, effective care
• Maintain accurate charting and ensure compliance with facility policies and regulatory standards

Qualifications

• Current LPN license in Pennsylvania or compact state eligibility as required
• Graduate of an accredited practical nursing program
• Basic Life Support (BLS) certification required or obtainable upon hire
• Strong clinical skills in medication administration, wound care, and resident assessment
• Excellent communication skills and ability to work well in a team-focused environment
• Prior long-term care or skilled nursing experience preferred but new grads are welcome to apply

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Solid understanding of infection control and safety procedures
• Comfortable with electronic charting and accurate documentation
• Ability to prioritize tasks in a fast-paced clinical setting
• Skilled at building rapport with residents and families to deliver person-centered care
• Competent in medication administration and basic clinical procedures
