Roles & Responsibilities
- Provide compassionate direct patient care consistent with LPN scope of practice
- Administer medications and document medication administration accurately
- Monitor patient condition and report changes to the RN or provider promptly
- Perform wound care, ADL assistance, and other clinical procedures as assigned
- Collaborate with the interdisciplinary team to develop and update patient care plans
- Maintain accurate, timely charting and follow facility policies and procedures
- Support a safe, respectful environment for patients, families, and coworkers
Qualifications
- Current Licensed Practical Nurse (LPN) license — Pennsylvania licensure required or eligible
- Basic Life Support (BLS) certification required; additional certifications a plus
- Comfortable with medication administration, basic assessments, and documentation
- Strong communication skills and the ability to work well within a care team
- Previous LPN experience preferred but new graduates from an accredited LPN program are welcome to apply
Education
- Graduate of an accredited LPN/LVN program
- High school diploma or GED
Knowledge & Skills
- Foundational nursing knowledge and clinical assessment skills
- Medication administration and safe documentation practices
- Familiarity with electronic health records (EHR) and charting
- Ability to prioritize tasks in a fast-paced clinical setting
- Compassionate patient communication and family engagement
