Roles & Responsibilities

• Provide compassionate direct nursing care to residents in a professional manner
• Administer medications and treatments accurately, following facility protocols
• Assess and document resident conditions, reporting changes to the care team
• Collaborate with RNs, CNAs and interdisciplinary team members to deliver person-centered care
• Perform and document routine clinical tasks such as vital signs, wound care, and ADL assistance
• Maintain accurate, timely electronic and paper records in accordance with facility standards
• Promote a safe environment and adhere to infection control and safety policies
• Support resident dignity and independence through respectful communication and care

Qualifications

• Current LPN license in good standing (required)
• CPR/BCLS certification (or willingness to obtain upon hire)
• Prior experience in long-term care or skilled nursing is preferred but not required
• Strong interpersonal skills and the ability to work effectively on a care team
• Ability to complete documentation accurately and follow clinical protocols

Education

• LPN diploma or certificate from an accredited nursing program

Knowledge & Skills

• Medication administration and accurate charting
• Basic clinical assessments and vital sign monitoring
• Experience with ADL support and compassionate resident care
• Familiarity with infection control practices and safety procedures
• Effective communication with residents, families and the healthcare team
• Comfort using electronic health records or bedside documentation systems
