Roles & Responsibilities
- Provide compassionate direct patient care, fostering dignity and comfort
- Administer medications and treatments safely and according to policy
- Perform routine assessments and monitor patient status and vital signs
- Document care accurately in the medical record and communicate changes promptly
- Collaborate with nurses, providers, and the care team to support excellent outcomes
- Support residents and families with clear communication and empathy
Qualifications
- Active Licensed Practical Nurse (LPN) license in Pennsylvania or eligible to obtain one
- Current CPR/BLS certification (or willingness to obtain upon hire)
- Comfortable working 12.5-hour shifts and flexible per diem scheduling
- Strong communication skills and ability to work as part of a team
- Previous experience in long-term care, skilled nursing, or similar setting preferred
Education
- LPN diploma or certificate and active state licensure required
Knowledge & Skills
- Medication administration and basic clinical assessments
- Accurate and timely documentation in electronic medical records
- Infection control, safety protocols, and resident-centered care
- Ability to prioritize tasks and manage time during busy shifts
- Good interpersonal skills for working with residents, families, and the care team
