Roles & Responsibilities
- Provide compassionate direct patient care including routine assessments and monitoring
- Administer medications and treatments safely and according to orders
- Monitor and document vital signs, patient responses, and clinical changes accurately
- Collaborate with the interdisciplinary care team to coordinate patient-centered plans
- Educate patients and families about care plans, medications, and discharge needs
- Escalate changes in patient condition promptly to appropriate clinical staff
- Maintain accurate, timely charting and uphold documentation standards
Qualifications
- Active Licensed Practical Nurse (LPN) credential — ability to practice in Pennsylvania required
- Current BLS/CPR certification or willingness to obtain upon hire
- Comfort with medication administration, basic assessments, and charting
- Ability to pass required background checks and health screenings
- Strong communication skills and a collaborative mindset
Education
- Graduate of an accredited Practical Nursing (LPN) program or equivalent diploma/certificate
Knowledge & Skills
- Solid knowledge of nursing fundamentals, vital sign assessment, and basic wound care
- Medication administration and documentation best practices
- Patient safety, infection control, and standard precautions
- Electronic medical record charting and basic computer skills
- Effective patient and family education techniques
