Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Monitor and report changes in patient condition to the nursing team promptly
• Assist with safe patient transfers, ambulation, and positioning
• Document care and observations accurately and timely
• Maintain a clean, safe, and therapeutic environment for patients
• Collaborate with the interdisciplinary team to support individualized care plans

Qualifications

• Current Certified Nursing Assistant (CNA) credential required
• Valid CNA certification/license in Pennsylvania or eligibility to obtain one
• Current CPR/BLS certification preferred or willingness to obtain
• Prior experience in direct patient care or long term care settings is a plus
• Strong communication skills and a team-oriented attitude
• Ability to pass required background checks and health screenings

Education

• High school diploma or GED preferred
• Completion of an accredited CNA training program and current CNA certification

Knowledge & Skills

• Knowledge of basic nursing procedures and infection control practices
• Skill assisting with ADLs including bathing, dressing, and feeding
• Ability to observe and report patient changes clearly and professionally
• Familiarity with safe patient handling and transfer techniques
• Basic charting and documentation skills
• Comfort working in a fast-paced, patient-focused healthcare environment
