Roles & Responsibilities

• Provide compassionate direct patient care to residents in the SNF, prioritizing dignity and respect
• Perform resident assessments and make rounds, noting changes in condition promptly
• Document assessments and care per facility policy to ensure clear continuity
• Communicate resident status and hand off accurate reports to nursing staff and team members
• Assist residents with activities of daily living and respond promptly to call lights
• Collaborate with families, physicians and interdisciplinary teams to support individualized care plans

Qualifications

• California CNA License — required (must have active, valid CA credential)
• Comfortable working in a Skilled Nursing Facility (SNF) environment
• Willingness to float between units and work weekends and holidays as needed
• Strong communication skills and a team-player attitude
• Experience with resident assessment and documentation preferred
