Roles & Responsibilities

• Provide compassionate, hands-on nursing care to assigned patients under the supervision of the nursing team
• Administer medications and treatments accurately and document in the medical record
• Monitor and record vital signs, recognize changes in condition, and promptly report concerns
• Assist patients with activities of daily living while promoting dignity and independence
• Collaborate with RNs, physicians and other disciplines to support coordinated care plans
• Maintain a safe, clean care environment and adhere to infection prevention protocols
• Educate patients and families about care plans, medications and follow-up instructions
• Contribute to efficient shift handoffs and clear, timely documentation

Qualifications

• Active Licensed Practical Nurse license (LPN) valid in New Hampshire
• Basic Life Support (BLS) certification required; additional certifications a plus
• Comfortable working in a fast-paced clinical setting and adapting to per diem assignments
• Strong communication and teamwork skills with a patient-first mindset
• Previous LPN experience in a hospital or long-term care setting preferred but motivated new grads welcome to apply

Education

• Graduate of an accredited Practical Nursing program or equivalent

Knowledge & Skills

• Solid fundamentals in medication administration and basic clinical assessments
• Proficient in documenting patient care accurately and clearly in an electronic medical record
• Good clinical judgment and ability to escalate concerns appropriately
• Time management and prioritization during busy shifts
• Effective interpersonal skills for working with patients, families and the care team
