Roles & Responsibilities

• Provide high-quality patient care by performing treatments and procedures according to established protocols
• Administer medications safely and document administration according to policy and state practice act
• Observe patients closely and report adverse reactions or significant changes in condition promptly
• Respond to patient requests in a timely, compassionate manner and escalate concerns to RN leadership as needed
• Participate in patient care planning and assist with implementing the plan under RN direction
• Deliver patient education and teach self-care strategies, documenting progress and outcomes
• Follow established nursing policies and initiate appropriate measures in emergency situations

Qualifications

• Active Illinois RN license (required)
• Current CPR certification (required)
• Experience in skilled nursing or long-term care settings preferred
• Comfort working 12.5-hour day shifts and participating in weekend/holiday rotations
• Strong clinical judgment, communication skills, and a commitment to patient-centered care

• Comprehensive patient assessment and monitoring
• Care planning and coordination within a skilled nursing/LTC setting
• Accurate and timely clinical documentation
• Patient education and engagement techniques
• Emergency response and critical thinking under pressure
