Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a rehabilitation setting
• Administer medications and document treatments accurately and timely
• Monitor and record vital signs, changes in condition, and communicate concerns to the care team
• Support individualized rehab plans by assisting with ADLs and therapeutic activities
• Collaborate with nurses, therapists, and physicians to coordinate excellent patient-centered care
• Maintain thorough, professional charting and ensure compliance with facility policies
• Educate patients and families about care plans and discharge instructions with clarity and empathy

Qualifications

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license — Maryland licensure preferred
• Current BLS (Basic Life Support) required; ACLS or other certifications a plus
• Prior experience in rehab, skilled nursing, or long-term care is preferred
• Strong clinical assessment, medication administration, and documentation skills
• Excellent communication and teamwork skills; reliability and a positive attitude

Education

• Graduate of an accredited practical nursing program for LPN (or associate/BSN program for RN applicants)

Knowledge & Skills

• Solid knowledge of medication administration and nursing fundamentals
• Familiarity with rehab care practices and assisting with activities of daily living
• Comfortable with electronic charting and accurate documentation
• Good clinical judgment, time management, and the ability to prioritize tasks
• Patient-centered communication and the ability to educate and support families
