Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a rehabilitation setting
- Administer medications and document treatments accurately and timely
- Monitor and record vital signs, changes in condition, and communicate concerns to the care team
- Support individualized rehab plans by assisting with ADLs and therapeutic activities
- Collaborate with nurses, therapists, and physicians to coordinate excellent patient-centered care
- Maintain thorough, professional charting and ensure compliance with facility policies
- Educate patients and families about care plans and discharge instructions with clarity and empathy
Qualifications
- Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license — Maryland licensure preferred
- Current BLS (Basic Life Support) required; ACLS or other certifications a plus
- Prior experience in rehab, skilled nursing, or long-term care is preferred
- Strong clinical assessment, medication administration, and documentation skills
- Excellent communication and teamwork skills; reliability and a positive attitude
Education
- Graduate of an accredited practical nursing program for LPN (or associate/BSN program for RN applicants)
Knowledge & Skills
- Solid knowledge of medication administration and nursing fundamentals
- Familiarity with rehab care practices and assisting with activities of daily living
- Comfortable with electronic charting and accurate documentation
- Good clinical judgment, time management, and the ability to prioritize tasks
- Patient-centered communication and the ability to educate and support families
