Roles & Responsibilities

• Provide compassionate, patient-centered care to residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and safely
• Monitor and document patient condition, vitals, and changes in status in the medical record
• Collaborate with RNs, interdisciplinary team members, and facility staff to coordinate care
• Support admissions, discharges and transitions with clear communication and follow-through
• Coach and educate patients and families on care plans, safety, and post-discharge needs

Qualifications

• Licensed Practical Nurse (LPN) — active license or ability to obtain New Hampshire licensure
• Travel LPN (T-LPN) experience or openness to travel assignments is a plus
• Current BLS/CPR certification preferred (or willingness to obtain)
• Comfort working a rotating schedule including weekends
• Strong clinical judgment, dependability, and ability to work as part of a team

Education

• Graduate of an accredited LPN/Vocational Nursing program

Knowledge & Skills

• Solid hands-on nursing skills in medication administration and wound care
• Proficient documentation and charting in electronic medical records
• Infection control best practices and safety-minded approach
• Good communication skills with patients, families, and clinical teams
• Time management and prioritization for long shift coverage
