Roles & Responsibilities

• Provide compassionate, skilled nursing care to residents in a long-term care setting
• Administer medications and treatments accurately and document thoroughly
• Assess resident status, monitor changes, and communicate concerns to the care team
• Collaborate with RNs, CNAs, and interdisciplinary staff to deliver personalized care plans
• Support safe admissions, discharges, and transitions of care with attention to detail
• Educate residents and families about care plans, medications, and health maintenance
• Maintain clear, timely charting in the facility’s clinical documentation system

Qualifications

• Current LPN license in New Mexico or eligible for state licensure
• Basic Life Support (BLS/CPR) certification (American Heart Association or equivalent)
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Ability to pass background check and drug screening as required
• Up-to-date immunizations and adherence to facility health requirements
• Strong communication skills and a team-first attitude

Education

• Completion of an accredited Practical Nursing (LPN/LVN) program or vocational nursing certificate

Knowledge & Skills

• Medication administration and accurate documentation
• Resident assessment and vital signs monitoring
• Familiarity with care planning and following physician orders
• Experience with wound care and basic clinical procedures
• Comfortable using electronic health records and charting tools
• Strong interpersonal skills for working with residents, families, and staff
