Roles & Responsibilities
- Provide compassionate, skilled nursing care to residents in a long-term care setting
- Administer medications and treatments accurately and document thoroughly
- Assess resident status, monitor changes, and communicate concerns to the care team
- Collaborate with RNs, CNAs, and interdisciplinary staff to deliver personalized care plans
- Support safe admissions, discharges, and transitions of care with attention to detail
- Educate residents and families about care plans, medications, and health maintenance
- Maintain clear, timely charting in the facility’s clinical documentation system
Qualifications
- Current LPN license in New Mexico or eligible for state licensure
- Basic Life Support (BLS/CPR) certification (American Heart Association or equivalent)
- Previous experience in long-term care, skilled nursing, or similar setting preferred
- Ability to pass background check and drug screening as required
- Up-to-date immunizations and adherence to facility health requirements
- Strong communication skills and a team-first attitude
Education
- Completion of an accredited Practical Nursing (LPN/LVN) program or vocational nursing certificate
Knowledge & Skills
- Medication administration and accurate documentation
- Resident assessment and vital signs monitoring
- Familiarity with care planning and following physician orders
- Experience with wound care and basic clinical procedures
- Comfortable using electronic health records and charting tools
- Strong interpersonal skills for working with residents, families, and staff
