Roles & Responsibilities

• Provide compassionate, skilled nursing care to assigned patients in accordance with facility policy
• Administer medications, treatments and injections as ordered and document accurately
• Monitor patient condition, recognize changes and communicate effectively with the care team
• Perform assessments, maintain accurate charts and support care planning
• Assist with admissions, discharges and patient education to promote positive outcomes
• Collaborate with RNs, therapists and support staff to ensure continuity of care
• Follow infection control protocols and facility safety guidelines to keep patients and staff safe

Qualifications

• Active LPN license (Licensed Practical Nurse) — Pennsylvania licensure preferred or eligible
• Graduation from an accredited practical nursing program
• Current Basic Life Support (BLS) certification or willingness to obtain prior to start
• Comfortable with 12+ hour day shifts and adaptable to facility staffing needs
• Strong communication, documentation and teamwork skills
• Previous experience in long-term care, skilled nursing, or acute care is a plus

Education

• Graduate of an accredited Practical Nursing (LPN) program; diploma or certificate

Knowledge & Skills

• Medication administration and accurate documentation
• Basic patient assessment and vital signs monitoring
• Wound care, ADL assistance and safe patient handling
• Infection prevention and control practices
• Effective, compassionate communication with patients and families
• Familiarity with electronic medical records and charting best practices
