Roles & Responsibilities
- Provide compassionate, hands-on nursing care to patients in a rehabilitation setting
- Administer medications and treatments accurately and timely
- Monitor patient status and communicate changes clearly to the interdisciplinary team
- Document assessments, interventions and patient progress in the electronic health record
- Support discharge planning and patient education to promote recovery and independence
- Collaborate with therapists and physicians to deliver coordinated, patient-centered care
- Maintain a safe, clean environment and adhere to facility policies and infection control protocols
Qualifications
- Active LPN or RN license (licensed in the state of Maryland or eligible for licensure)
- Prior experience in rehab, long-term care, or similar clinical setting preferred
- Current Basic Life Support (BLS) certification preferred or willing to obtain
- Strong clinical assessment and medication administration skills
- Excellent communication and teamwork skills with a patient-first attitude
Education
- LPN: Graduate of an accredited practical nursing program (certificate/diploma)
- RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Proficient in medication administration and wound care basics
- Skilled at patient assessment, vital sign monitoring and documentation
- Comfortable working with interdisciplinary rehab teams
- Familiarity with electronic health record charting and care planning
- Good time management and ability to prioritize in a fast-paced setting
