Roles & Responsibilities

• Provide compassionate, hands-on nursing care to patients in a rehabilitation setting
• Administer medications and treatments accurately and timely
• Monitor patient status and communicate changes clearly to the interdisciplinary team
• Document assessments, interventions and patient progress in the electronic health record
• Support discharge planning and patient education to promote recovery and independence
• Collaborate with therapists and physicians to deliver coordinated, patient-centered care
• Maintain a safe, clean environment and adhere to facility policies and infection control protocols

Qualifications

• Active LPN or RN license (licensed in the state of Maryland or eligible for licensure)
• Prior experience in rehab, long-term care, or similar clinical setting preferred
• Current Basic Life Support (BLS) certification preferred or willing to obtain
• Strong clinical assessment and medication administration skills
• Excellent communication and teamwork skills with a patient-first attitude

Education

• LPN: Graduate of an accredited practical nursing program (certificate/diploma)
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Proficient in medication administration and wound care basics
• Skilled at patient assessment, vital sign monitoring and documentation
• Comfortable working with interdisciplinary rehab teams
• Familiarity with electronic health record charting and care planning
• Good time management and ability to prioritize in a fast-paced setting
