Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a rehab setting
• Administer medications and treatments accurately and document interventions thoroughly
• Monitor and assess patients' clinical status and report changes to the interdisciplinary team
• Support rehabilitation goals by assisting with activities of daily living and care plans
• Collaborate with therapists, case managers, and physicians to coordinate safe discharges and transitions
• Maintain accurate, timely charting and follow facility policies and regulatory requirements
• Promote a safe environment by identifying and addressing potential safety concerns
• Build trusting relationships with residents and families through clear, empathetic communication

Qualifications

• Current, active LPN license valid in New Mexico (required)
• Previous practical nursing experience in rehab, long-term care, or skilled nursing preferred
• Basic Life Support (BLS/CPR) certification (or willingness to obtain upon hire)
• Ability to pass background checks, drug screening, and required health screenings
• Strong teamwork, communication, and time-management skills
• Comfort with overnight shift responsibilities and working independently when needed

Education

• Completion of an accredited Practical Nursing program (LPN/LVNs diploma or certificate)

Knowledge & Skills

• Solid clinical knowledge of medication administration and wound care principles
• Skilled in patient assessment, documentation, and care planning
• Familiarity with electronic medical records and accurate charting practices
• Ability to prioritize tasks in a fast-paced rehab environment
• Compassionate bedside manner and strong interpersonal skills
• Basic knowledge of infection control and safety procedures
