Roles & Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans that drive meaningful recovery
- Deliver evidence-based therapy interventions to improve mobility, strength, balance, and independence
- Complete evaluations, progress notes, and discharge documentation in line with facility and regulatory standards
- Collaborate with nurses, therapists, and other team members to ensure coordinated, patient-centered care
- Educate patients and families on therapy goals, safety, and home exercise programs to support long-term outcomes
- Adapt treatment approaches based on patient progress and changing clinical needs
Qualifications
- Active California Physical Therapist license in good standing
- Graduate of an accredited Physical Therapy program
- Skilled nursing or rehabilitation experience preferred
- Strong clinical reasoning, communication, and documentation skills
- Ability to work independently while contributing positively to a multidisciplinary team
Education
- Graduate of an accredited Physical Therapy program
Knowledge & Skills
- Physical therapy assessment and patient evaluation
- Individualized treatment planning and rehabilitative care
- Evidence-based therapeutic interventions for mobility and strength
- Clear, timely documentation and progress note preparation
- Effective interdisciplinary communication and patient/family education
- Familiarity with skilled nursing facility workflows and regulatory expectations
