Roles & Responsibilities

• Perform thorough patient evaluations to create individualized treatment plans
• Deliver daily therapeutic treatments that help patients regain independence
• Coordinate safe and timely discharges to support rehab-to-home outcomes
• Document care and outcomes accurately and efficiently following facility policy
• Track productivity metrics and meet departmental productivity expectations
• Collaborate with therapists, nursing staff, and families to optimize patient progress
• Adhere to facility dress code and professional standards while representing the rehab team

Qualifications

• 1-3 years of Occupational Therapy experience (posted requirement: 1-3 years)
• Prior SNF (skilled nursing facility) experience strongly preferred
• Current Occupational Therapy license as required by state/facility (confirm before start)
• Ability to be on-site for scheduled shifts and meet productivity expectations
• Comfort following facility policies (CRG P & P) and working within a team-based rehab environment

Knowledge & Skills

• Skilled in patient evaluations and creating effective OT treatment plans
• Experience delivering daily treatments and managing caseloads in a SNF setting
• Familiarity with discharge planning and rehab-to-home strategies
• Productivity tracking and meeting performance metrics
• Strong communication and teamwork skills to coordinate interdisciplinary care
